• Up to three e-mailed invitations asking patients to complete the survey online.
• If no online response is received, the patient receives up to two mailed paper surveys to complete and return.

This hybrid approach offers the best of both worlds – giving patients the convenience of responding online while still capturing feedback from those who prefer a traditional paper survey.
